"A model release is a legal contract, and like any other legal form, it cannot be changed or altered in any way once it is signed. Adding dates, descriptions, or any other information after the model has signed an otherwise blank release is not acceptable, the info, especially dates and shoot description need to be completed at the same time the model signs the form.
